  9. The name of the file must include the category initials (SP for suicide prevention and MI for mental illness stigma reduction), your school's initials and the name of the county your school is located in. For example

  18. All music in this entry is royalty free or permission to use the copyrighted music has been granted by the copyright holder for this film to be distributed in a pubic marketplace and the permission form is in our possession.
  19. All necessary release forms are signed and on file with the student advisor.
  20. Each student associated with this entry MUST read the release and indicate agreement by "collectively" checking the designated box below.

Release *I/we, the above named student(s) am/are the author, owner, and/or copyright holder (hereafter “I” or "me") of, and hereby grant permission for the Slick Rock Student Film Festival (SRSFF) to include, the above-named material (the Material) in any and all forms, in the above-mentioned venue. I further allow SRSFF to distribute the Material as part of the above. I also grant SRSFF the right to publish this submission as part of the SRSFF Web site or other website used or created by SRSFF, on DVD-ROM, multimedia or any other media format now or hereafter known. I also grant to SRSFF the right to use my name and likeness in connection with the publication and distribution of the Material. In the event that any elements used in the Material contain the work of third-party individuals, I understand that it is my responsibility to secure any necessary permissions and/or licenses and will provide them in to SRSFF if requested. I hereby warrant that I have the exclusive right to grant the rights referred to herein and that the Material shall be free from libelous, infringing, or other unlawful matter. I agree to indemnify SRSFF against any loss, injury or damages (including (without limitation) legal costs or expenses properly incurred) occasioned to SRSFF in consequence of any breach of these warranties. I hereby release and discharge SRSFF and associated sponsors and organizers from any and all liability arising out of my inclusion in the publication or in connection with the performance of any of the activities described in this entry as permitted herein. This includes, but is not limited to, my right of privacy or publicity, copyright, patent rights, trade secret rights, moral rights or trademark rights. All permissions and releases granted by me herein shall be effective in perpetuity, and extend and apply to SRSFF and its assigns, contractors, sublicensed distributors, successors and agents.
